Available to students entering grades 1-12

1. Please fill out the New Student Registration Form.
2. The following items will be needed upon registration:
   • Birth Certificate (Hospital records will not be accepted)
   • Social Security Card
   • Immunization Record
   • Insurance/Medicaid Card

After completing the forms, please return them to your child's campus. Forms may be completed at their campus.
*Home Language Survey is available in Arabic, Chinese, Hmong, Laotian, Marshallese, Tamil, Telugu, and Vietnamese upon request.
